  • Under the supervision of a Registered Nurse, the LPN/LVN(Licensed Practical/Vocational Nurse) - Home Health provides skilled one-on-onenursing care to patients in their home and accompanies them when indicated toappointments, etc.

    Essential Duties and Responsibilities

    • Documents each shift or visit to reflect accurately theprovision of skilled nursing care rendered in accordance with physician'sorders.

    • Changes in patient condition are identified and documentedwith resolution on the appropriate clinical form. Director/Supervisor notification if applicable

    • Physician notification to obtain necessary orders forintervention(s) per state regulations

    • Performs specific treatments and medication administrationin accordance with physician orders and home health agency's protocols

    • Documentation reflects patient collaboration andparticipation in their plan of care

    • Requests for modification of orders and additional ordersare made based on the patient's clinical status and skilled nursing judgment

    • All physician collaboration/coordination of care isdocumented in the medical record and communicated as needed

    • Provides patient and family education as appropriate

    • Attends case conferences, team meetings and educationin-services as required

    Minimum Requirements

    • Graduate of an accredited practical or vocational nursingprogram

    • Currently licensed as a Licensed Practical/Vocational Nursein the state in which the LPN/LVN will practice

    • One year prior professional nursing experience, home careexperience preferred

    • Complies with all relevant professional standards ofpractice

    • Participation and completion of Amergis' Competency programwhen applicable

    • Current CPR if applicable

    • TB questionnaire, PPD or chest x-ray if applicable

    • Current Health certificate (per contract or stateregulation)

    • Must meet all federal, state and local requirements

    • Successful completion of new hire training as applicable tojob site

    • Understand patient confidentiality and HIPAArequirements

    • Ability to effectively elicit/provide information to andfrom appropriate individuals (including, but not limited to, supervisors,co-workers, clients) via strong communication skills; proficiency in theEnglish language is required

    • Computer proficiency required

    • Must be at least 18 years of age
