  • UPMC's Transition of Care department is hiring a Licensed Practical Nurse (LPN)! This is a full time position, with flexible start times after 8:30am, and the opportunity to work from home after a 6 month training period!

    Responsibilities in this role include making post hospital discharge calls, completing medication reconciliation with patients, scheduling patients for Primary Care for hospital follow ups, escalating patient concerns, and complying with all department policies and procedures.

    This position is based out of the Lemoyne, PA offices and you must live within 90 minutes of Lemoyne, PA

    Purpose:

    Provides professional nursing care to patients under supervision of patient care providers.

    Responsibilities:

    • Comply with requirements of appropriate regulatory agencies (i.e. OSHA, CLIA, PA Department of Health).

    • Make post hospital discharge calls

    • Complete medication reconciliation with each patient

    • Schedule patients with their PCP for hospital follow up

    • Escalate patient concerns

    • Comply with all policies and procedures.

    • Document all activities/interventions, patient/family responses, medication dispensed/prescribed, etc., in the medical record.

    • Maintain strict confidentiality related to medical records and other data.

    • Promote wellness by providing patient education materials, communicating physician advice/instructions.

    Graduate of an approved LPN school of nursing. One year clinical experience preferred.

    Licensure, Certifications, and Clearances:

    CPR required based on AHA standards that include both a didactic and skills demonstration component within 30 days of hire

    Clearances must be dated within 90 days

    • Advanced Cardiac Life Support (ACLS) OR Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR)

    • Licensed Practical Nurse (LPN)

    • Act 34 *Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
